  • The Matching Gift Program is for donors giving either a one time gift or a yearly total of $900.00 or more. For example, if you join the Colonel's Club above and pledge $75.00 per month, your total would be $900.00. You could then use that amount as a matching pledge. If you would like to participate and are giving a total of $900.00 or more...please check the appropriate box below.
  • After filling out the form, Please Click the Submit Button so we will have an immediate record of your gift. Then you can choose your method of payment as outlined below. NEW PARTNERS: You should receive a welcome letter 2-3 weeks after Share-A-Thon. Enclosed will be an envelope to return your gift. EXISTING PARTNERS: Please continue to use the pre-addressed envelopes found in your PathPointe Magazine. CREDIT CARDS: For immediate gifts, click "Donate" on the next page to use PayPal to make your donations.
